How it works
Betaine salicylate is a salt/ester complex of salicylic acid with betaine — trimethylglycine — developed as a gentler alternative source of BHA activity and notably popularised in some Korean cosmetic formulations. It is synthetically manufactured. The intended behaviour is that it delivers salicylate's keratolytic and anti-inflammatory benefits with a milder release profile and less of the direct acid sting associated with free salicylic acid. For context, salicylic acid itself is lipid-soluble, penetrates into the sebum-filled follicle, dissolves the intercellular lipids holding corneocytes together inside the pore, and carries anti-inflammatory activity through COX inhibition — that is the mechanism betaine salicylate is meant to deliver more gently.

This is where honesty matters. Independent, large-sample peer-reviewed clinical trials specifically isolating betaine salicylate's efficacy against free salicylic acid are limited in the PubMed-indexed literature.
